You can access the application form by clicking on the link below OR contact the NBDA Tournament Directors at nbdainfo@yahoo.com OR you can contact your area coach / contact person and they will email you the attachment (a pdf file). After you have filled out the form, you should:
• Print a copy of the document (you will not be able to save it).
• Sign the printed document and bring it with you to the team try-out -- notify the coach / contact person that you have the document or mail it to them.
• Click on the "Submit" button to send a copy to the NBDA
NOTE: The "Submit" button will NOT work if you are using a web-based email application such as yahoo or hotmail -- you need to be using a desktop email application such as "Mail" on a mac, or "Outlook" on a PC.
